How to Prepare for Admissions at House of Hope
Complete an assessment by calling Assessment and Referral Services (ARS) for an appointment. You don’t need insurance for an assessment. If you’ve had an assessment in the past six months, call us for guidance.

Call our Admissions Specialist at (801) 487-3276 x1233, between Monday and Friday from 8:30 am to 5 pm, and let them know that you’ve completed an assessment.
We’ll call you to schedule an intake appointment once we receive your assessment. You are welcome to call us any time with questions you may have. Please note that House of Hope is currently unable to take walk-ins.
Gather needed documents. When you come in for your intake appointment, you’ll need your Utah ID or driver’s license, birth certificate, and social security card, if you have them. Need help getting these items? Click below!
